    Deadline Details

    Applications are accepted throughout the year. Initial approach should be with a Letter of Intent on organization's letterhead. An application form is required. Applicants should submit the following:
    • Copy of most recent annual report/audited financial statement/990
    • Descriptive literature about organization
    • Copy of IRS Determination Letter
    • Signature and title of chief executive officer
    • Detailed description of project and amount of funding requested
