    Summary

Through our grantmaking, The Connecticut Health Foundation supports organizations and institutions that seek to implement systems-change solutions. By bringing funds and strategic thinking together, we can make greater strides toward changing our health system to create better outcomes all across Connecticut. As part of their investment, they fund projects and initiatives that support the concept of health equity.

The Foundation solicits grant proposals through Requests for Proposals (RFPs), and welcome proposals with applicant project ideas, as well. They also give out various small grants each year from our Presidents Discretionary Grant Award.

Expanding health equity has always meant creating more access to better care, especially for those who currently lack it now”people of color. This has become a funding priority for the Foundation. This means helping people
    • understand how their health insurance works
    • navigate our complicated system of care when they need to
    • find a suitable medical home where they can confidently go for care.
